Southeast Missouri REALTORS® to Host Flu Clinic

Description

Southeast Missouri REALTORS® will be hosting MediCenter for an on-site vaccine clinic on Monday, October 5th from 9 am – 10:30 am. MediCenter will have the quadrivalent vaccine and will also bring the high dose vaccine for anyone 65+. 

What will you need to bring the day of the clinic: 

1. Your completed screening questionnaire and consent form. (Questions 1 – 8 only) - once you register, staff will email this to you. 

2. Commercial Insurance card. Please be sure that the insurance card has the pharmacy benefits listed (RX BIN, etc.). If there is no insurance, the discounted cash price is $30. 

3. For individuals 65+, please bring the red, white, and blue Medicare card. Your vaccine is 100% covered thru Medicare.
